Sulfametoxazol inhibe de forma competitiva la utilización del ácido para-aminobenzoico (PABA) en la síntesis del dihidrofolato que se produce en la célula bacteriana produciendo bacteriostasis. Trimetoprima inhibe de forma reversible la dihidrofolato reductasa bacteriana (DHFR), una enzima activa en la ruta metabólica del folato, que convierte dihidrofolato en tetrahidrofolato. Dependiendo de las condiciones, el efecto podría ser bactericida. Así, trimetoprima y sulfametoxazol bloquean dos etapas consecutivas en la biosíntesis de purinas y por lo tanto, ácidos nucleicos esenciales para muchas bacterias. Esta acción, produce una potenciación marcada de la actividad in vitro entre los dos agentes.
